29 CFR 1910.134(d)(3)(iii)(B)(2): The employer did not implement a change schedule for respirators not equipped with an End of Service Life Indicator that would ensure cartridges were changed before the end of their service life:    (a)     The 3M full face negative pressure respirator used at the top coat paint booth did not have the cartridges changed following the cartridge change schedule.      Note:  The employer may provide an air-purifying respirator provided the respirator is equipped with end-of-service-life indicators or a change schedule has been developed.  Otherwise, the employer must provide an atmosphere-supplying respirator.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    (a)     The iron worker, an Original Peddinghaus, used to punch holes in metal bars, had no guard at the punch.    (b)     The iron worker, an Original Peddinghaus, used to shear metal, had a partially unguarded shear blade and the fixed metal barrier guard was missing fasteners and was loose.   (c)     The approximately 16 torque machines in the MSA area had rotating parts with no guard to prevent employee exposure.
