29 CFR 1910.23(b)(2): Every ladder rung, step, and/or cleat is not spaced less than 10 inches (25 cm) and no more than 14 inches (36 cm) apart, as measured between the centerlines of the rungs, cleats, and steps:    a) Conveyor Belt Catwalk; On or about January 5, 2017, and at times prior, the employer exposed employees to fall hazards, in that the fixed ladder used to access a catwalk was installed with rungs measured to be 16 inches apart.
