29 CFR 1904.39(a)(2): The employer failed to report the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, within (24) twenty-four hours Wal-Mart Stores, Inc. Store #3621, 1871 Chamblee Tucker Rd, Chamblee, GA - On September 20, 2016, an employee was hospitalized after losing consciousness at the store parking lot while pushing carts. The employer did not notify OSHA of the hospitalization within 24 hours, on or about 12/27/16. Wal-Mart Stores Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1904.39(a)(2), which was contained in OSHA inspection number 11308506, citation number 1, item number 1 and was affirmed as a final order on 04/07/16, with respect to a workplace located at 65 Wal-Mart Dr., Mountain Home AZ, 72653. Wal-Mart Stores Inc. was also previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1904.39(a)(2), which was contained in OSHA inspection number 1079441, citation number 2, item number 1 and was affirmed as a final order on 06/06/16, with respect to a workplace located at 1501 Skyland Blvd, Tuscaloosa, AL 35405.
