29 CFR 1926.602(d): The employer did not certify that each operator had been trained and evaluated as required by paragraph 29 CFR 1910.178(l): a) Worksite - On or prior to January 6, 2017, the employer did not train, evaluate and certify a forklift operator prior to the employee operating a Skyworks LLC rented forklift 5709 (2011 Heli CPYD25; serial number 230250N0208) exposing employees to struck-by and caught-between hazards. Note: The requirements applicable to construction work under paragraph (d) of this section are identical to those set forth at paragraph (l) of 29 CFR 1910.178.
