29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered:  Employees were exposed to chemical burns to their skin from battery acid. Forklift operators were exposed to chemical burns when adding water to the forklift battery. The employer did not provide personal protective equipment, such as an anti-overflow device, gloves, face shield, apron and rubber gloves when servicing the forklift battery.

29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):  Employees in the warehouse were exposed to struck by hazards.   Forklift operators were moving materials in the warehouse. A forklift employee and a temporary employee had not received forklift training prior to operating the Toyota electric forklift.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:  Employees were exposed to hazardous chemicals. The employer did not develop or implement a Hazard Communication program.  Employees throughout the facility were exposed to hazardous chemicals such as but not limited to battery acid.
