29 CFR 1910.179(m)(1): Thorough monthly inspections of rope conditions, with written, dated and signed reports, were not performed on the running ropes: a) Crane #1, New Car Manufacturing Area, 1477 DeGol Industrial Drive, Hollidaysburg, PA 16648: On or before November 15, 2016, a certification of record including the date of inspection, the signature of the person who performed the inspection and an identifier for the ropes which were inspected were not maintained for the 25 ton overhead crane (ID # BC01) located in the New Car Manufacturing Area. b) Crane #5, Manufacturing Area, 1477 DeGol Industrial Drive, Hollidaysburg, PA 16648: On or before November 15, 2016, a certification of record including the date of inspection, the signature of the person who performed the inspection and an identifier for the ropes which were inspected were not maintained for the 25 ton overhead crane (ID # BC05) located in the Manufacturing Area.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Shop area, 1477 DeGol Industrial Drive, Hollidaysburg, PA 16648: Observed on or about December 12, 2106, a Primeline brand hydraulic press brake (model number 3702, serial number 9682, 200 ton capacity) was being used without point-of-operations guards. Employees operating the press brake were exposed to crushed-by hazards. b) Shop area, 1477 DeGol Industrial Drive, Hollidaysburg, PA 16648: Observed on or about December 12, 2106, a Hurco brand hydraulic hydraulic press brake (serial number HFLA9031064B, 600 ton capacity) was being used without point-of-operations guards. Employees operating the press brake were exposed to crushed-by hazards.
