On or about January 6, 2017, employees sheathing the floor deck of a 2 story residential construction project were not protected by any means of personal fall protection, guardrails or safety nets, and were exposed to an 11 foot fall hazard. b. On or about January 6, 2017, Employees constructing an exterior wall of a two story residential project, were not protected from falling through holes more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around the floor opening.
