29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, were not protected by protective helmets.    On or about 5 January, 2017 and at times prior thereto, employees were observed working at a residential home without protective helmets in areas where falling object hazards were present exposing the employees to head injuries.

29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.     On or about 5 January 2017 and at times prior thereto, employees were observed using pneumatic nail guns on the siding of a residential home, exposing the employees to the hazard of being struck by flying objects.

On or about 5 January, 2017, and at times prior thereto, the employer was working on a scaffold that was not designed by a qualified person and that was not constructed and loaded in accordance with that design, exposing the employees to a falling hazard.

29 CFR 1926.501(b)(1): Unprotected sides or edges that were 6 feet or more above a lower level were not protected from falling hazards:    On or about January 5, 2017, and at times prior thereto, the employer did not equip a stairway landing and second level housing landing with fall protection.  Workers were exposed to falling more than 6 feet to a lower level due to the lack of guardrails or other fall protection devices.
