On or about January 10, 2017, Fabrication Shop: In the following locations, exits were not provided with illuminated exit signs:  1.  Rear exit door near the garage bay. 2.  Side of the shop.  B.  On or about January 10, 2017, Warehouse:  The exit in the front of the warehouse was not provided with an illuminated exit sign.

On or about January 3, 2017, Fabrication Shop:  The electrical power cordset on the Automaire Fan Company 31-inch metal-encased pedestal fan used for ventilation had damaged insulation and exposed wiring, exposing employees to electric shock hazards.

On or about January 10, 2017, Fabrication Shop:  In the following locations, wall-mount listed electrical outlet boxes were used to provide pendant power, exposing employees to electric shock hazards:  1.  Fabrication Shop, Rear:  A wall-mount listed 220-V outlet box with perforated knockouts, identified as A 3/5, was used to energize the Victaulic Tool Company VE-268 Series grooving machine.  2.  Fabrication Shop, Rear:  A wall-mount listed two-receptacle 110-V outlet box with perforated knockouts, identified as A1, was used to power fans and lights in the area.  3.  Fabrication Shop, Side:  A wall-mount listed 220-V outlet box with perforated knockouts was used to power a Millermatic 250 welding machine.

On or about January 23, 2017, Fabrication Shop:  The work space in front of the Siemens G4242ML3200CU 3 Phase, 3 Wire, 240V Indoor Load Center was blocked by the storage of three metal pipes; two 5-gallon containers of Valvoline HP 85W-140 Gear Oil; one 5-gallon container of Scales Air Compressor Corporation RE680 Compressor Oil; and two containers of Quikrete Exterior Use Anchoring Cement.  In case of electrical emergency, access to the electrical panel would be delayed.

29 CFR 1904.4(a): The employer did not record each work-related fatality, injury or illness case that resulted in the general recording criteria on the OSHA Form 300 or equivalent:    Northern Windustrial Co.:  On or about January 10, 2017, the employer did not record the following workplace injuries or illnesses which met the general recording criteria on the OSHA Form 300 or equivalent for the calendar year 2016:    a) On or about February 5, 2016, an employee sustained an elbow injury which resulted in days away from work and restricted duty.  The injury would be appropriately captured with a check in Column H (Days Away), 34 days away in Column K, 92 days restriction in Column L, and a check in Column M1.    b) On or about May 20, 2016, an employee sustained a laceration to the right lower leg which resulted in restricted duty.  The injury would be appropriately captured with a check in Column I (Restricted Duty/Transfer), 9 days restriction in Column L, and a check in Column M1.    c) On or about August 1, 2016, an employee sustained an injury to the left lower back which resulted in days away and restricted duty.  The injury would be appropriately captured with a check in Column H (Days Away), 3 days away in Column K, 3 days restriction in Column L, and a check in Column M1.    d) On or about September 14, 2016, an employee sustained an injury to the right elbow which resulted in restricted duty.  The injury would be appropriately captured with a check in Column I (Restricted Duty/Transfer), 4 days restriction in Column L, and a check in Column M1.    e) On or about September 29, 2016, an employee sustained a laceration injury to the right hand and thumb which resulted in restricted duty.  The injury would be appropriately captured with a check in Column I (Restricted Duty/Transfer), 2 days restriction in Column L, and a check in Column M1.    Abatement Certification and Documentation is required
