29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who was on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems.  a) 4050 Peppers Ferry Rd, Radford, VA 24141, Boiler Building: On or about December 13, 2016, and times prior thereto, employees that performed work to install metal decking with up to an approximately 32'7"fall distance from eave to lower level that included boiler structures, equipment and the finished floor, were not protected from fall hazards by the use of guardrail systems, safety net systems, or personal fall arrest systems, exposing the employees to fall hazards.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.502(d)(15): Anchorage used for attachment for personal fall arrest equipment was not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached nor was it designed, installed and used as follows: As part of a complete personal fall arrest system which maintains a safety factor of at least two; and under the supervision of a qualified person.  a) 4050 Peppers Ferry Rd, Radford, VA 24141, Boiler Building: On or about December 13, 2016, and times prior thereto, employees that performed work to install metal decking with up to an approximately 32'7" fall distance from eave to lower level, used an anchorage that was not designed, installed and used as part of a complete personal fall arrest system that maintained a safety factor of at least two; and under the supervision of a qualified person, exposing the employees to fall hazards.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level.  a) 4050 Peppers Ferry Rd, Radford, VA 24141, Boiler Building: On or about December 13, 2016, and times prior thereto, employees that performed work to install metal decking with up to an approximately 32'7" fall distance from eave to lower level, used Fall Tech 7276WR 20' Web SRLs, that were rigged to an anchor at foot-level such that employees could free fall greater than 6 feet to a lower level, exposing the employees to fall hazards.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
