29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control: On or about January 9, 2017, an employee was observed to be making adjustments inside of a Fadal VMC6535 Vertical Milling Machine (sn: 12002124926) without proper lockout procedures being utilized. The employee had not been listed as an authorized employee and has not received authorized training.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedure) was not done in sequence as required by 29 CFR 1910.147(d)(1) through (d)(6): On or about January 9, 2017, employees were exposed to machine hazards associated with moving parts while performing setup procedures on a Fadal VMC6535 Vertical Milling Machine (sn: 12002124926). Employees were observed to be inside of the mill performing a routine task directly at the point of operation. The employer failed to implement energy control application steps as the machine was not shut down or turned off to perform the centering procedure [per the 1910.147(d)(2) requirements]. As a result, the remaining applicable energy control elements, involving machine isolation [(d)(3)], LOTO device application [(d)(4)], dissipation of residual energy [(d)(5)(i)] and verification of isolation [(d)(6)], were not implemented to protect employees from machine setup hazards. prepare for shutdown [(d)(1)] - ensure authorized employee has knowledge of the type, magnitude, the hazards, and the method or means to control the energy; shutdown the machine or equipment [(d)(2)]; machine isolation [(d)(3)] Physically locate and operate all energy isolating devices that are needed to control the energy to the machine or equipment in such a manner as to isolate the machine or equipment from the energy source(s); LOTO device application [(d)(4)] - Affix a lockout or tagout device to each energy isolating device; dissipation of residual energy [(d)(5)(i)] - relieve, disconnect, restrain, and otherwise render safe all potentially hazardous stored or residual energy; verification of isolation [(d)(6)] - Verify that isolation and deenergization of the machine or equipment has been accomplished

On or about January 9, 2017, employees were exposed to the point of operation on Fadal VMC4020 (Shop #1170). The equipment did not have a functional interlock in place, allowing doors to be opened during the machine cycle. b. On or about January 9, 2017, employees were exposed to the point of operation on Fadal VMC6535 (Shop #1107). The equipment did not have a functional interlock in place, allowing doors to be opened during the machine cycle. c. On or about January 9, 2017, employees were exposed to the point of operation on Fadal VMC6030 (Shop #1138). The equipment did not have a functional interlock in place, allowing doors to be opened during the machine cycle. d. On or about January 9, 2017, employees were exposed to the point of operation on Fadal Vertical Milling Machine (Shop #1135). The equipment did not have a functional interlock in place, allowing doors to be opened during the machine cycle. e. On or about January 9, 2017, employees were exposed to the point of operation on Fadal VMC4020 (Shop #1134). The equipment did not have a functional interlock in place, allowing doors to be opened during the machine cycle. f. On or about January 9, 2017, employees were exposed to the point of operation on Fadal VMC4020 (Shop #1169). The equipment did not have a functional interlock in place, allowing doors to be opened during the machine cycle. g. On or about January 9, 2017, employees were exposed to the point of operation on Fadal VMC4020 (Shop #1188). The equipment did not have a functional interlock in place, allowing doors to be opened during the machine cycle. h. On or about January 9, 2017, employees were exposed to the point of operation on Fadal VMC4020 (Shop #1187). The equipment did not have a functional interlock in place, allowing doors to be opened during the machine cycle. i. On or about January 9, 2017, employees were exposed to the point of operation on Fadal VMC4020 (Shop #1077). The equipment did not have a functional interlock in place, allowing doors to be opened during the machine cycle.
