29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): On or about October 2nd, 2017, the employer failed to ensure that the 1052 Chevalier 1236 surface grinder was guarded to protect employees from reaching the point of operation while the grinding wheel was in operation. No guarding was in place during operation. Hycomp LLC was previously cited for a violation of this occupational safety and health standard 29 CFR 1910.212(a)(3)(ii), which was contained in OSHA inspection number 1201361, citation number 1, item number 1c and was affirmed as a final order on February 27, 2017, with respect to a workplace located at 17960 Englewood Drive, Cleveland, OH.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area: On or about September 23, 2017, the employer failed to ensure that employees received training in the safe use and handling of hazardous chemicals in the workplace prior to working with them. Hazardous chemicals include but are not limited to coolants and lubricants.
