29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:    On or about January 9, 2017, the employer failed to conduct at least an annual inspection of the lock out tag out procedures and ensure that it met all of the requirements within the standard.  This standard was was referenced in their Lock Out Tag Out Procedures.

The lathe did not have a chuck guard in place.

29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch:  On or about January 9, 2017, the employer failed to ensure that the work rest on the bench grinder (American Forge Foundry, serial #12044), located in the main shop at the Sidney lathe work station, was adjusted such that the distance from the work rest to the grinding wheel was within the measurement of 1/8 inch.  Measurements taken on site showed the distance from the grinders work rest to the grinding wheel measuring approximately 1/2 inch.

29 CFR 1910.242(b): Compressed air used for cleaning purposes was not reduced to less than 30 p.s.i.:    On or about January 9, 2017, the employer failed to ensure that a compressed air hose, that was being used by the Sidney Machine Co. lathe in order to clean off the lathe, misc. parts and other debris, was designed such that it was able to control the flow of air, keeping the flow at no more than 30 psi.  The hose was tested onsite and  registered a reading in excess of 90 psi.

29 CFR 1910.243(c)(1): Abrasive wheel(s) were used on portable grinder(s) which were not provided with safety guard(s) meeting the requirements specified in 29 CFR 1910.243(c)(1) through (c)(4):    On or about January 9, 2017, the employer failed to ensure that a portable powered angle hand grinder (Dewalt, serial # 260041), had a safety guard in place to protect the employee who was operating the grinder at the Sidney lathe, from coming into contact with the abrasive wheel when the hand grinder was in operation.
