29 CFR 1910.176(b): Material stored in tiers was not stacked, blocked, interlocked or limited in height so that it was stable and secure against sliding and collapse: a. On or about January 4, 2017, in the records storage unit, employees depositing and retrieving paper records files for the purchasing and quality assurance departments were exposed to collapse and crushing hazards from the 10 feet tall shelving systems that had been overloaded and modified to support wall paneling and hinged and sliding steel scissor gates.
