29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes. On or about January 4, 2017 and times prior thereto, the employer did not ensure employees working on a 2 in 12 sloped roof were protected from 13 feet falls, including through skylights.

29 CFR 1904.39(a)(2): Basic Requirement. Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA. On or about January 4, 2017 and times prior thereto, the employer failed to report an inpatient hospitalization of an employee resulting from a work-related fall to OSHA within twenty- four (24) hours.
