29 CFR 1926.501(b)(1): Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:      a) Job site, 7451 McKnight Road, Pittsburgh, PA: On or about January 9, 2017, during construction activities, the employer did not ensure that employees used appropriate fall protection systems/devices while walking on open floor joists where an approximate 15 foot fall hazard existed.     b) Job site, 7451 McKnight Road, Pittsburgh, PA: On or about January 9, 2017, during construction activities, the employer did not ensure that employees used appropriate fall protection systems/devices while walking on an open sided platform along the upper portion of the building where an approximate 15 foot fall hazard existed.

29 CFR 1910.178(l)(4)(ii)(A): The employer did not provide refresher training when the operator had been observed to operate the vehicle in an unsafe manner:   a) Job site, 7451 McKnight Road, Pittsburgh, PA: On or about January 9, 2017, during construction activities, the employer did not ensure that a forklift driver was provided refresher training where work operations were conducted in an unsafe manner. Deficiencies noted during the inspection included, but were not limited to, failing to inquire with the vehicle manufacturer to see if the unit could be used to lift people up in a basket and if they could use their homemade basket to lift people up on the unit, not ensuring that the basket platform was properly secured to the forks and allowing the driver of the unit to leave the operators cab while employees were raised up in the basket on the forks.

29 CFR 1926.405(j)(2)(ii):  Receptacles installed in wet or damp locations were not designed for that location:   a) Job site, 7451 McKnight Road, Pittsburgh, PA: On or about January 9, 2017, during construction activities, the employer did not ensure that electrical outlets were appropriate for the work environment in that outlets were placed outside the building, without any covers or protection from the elements and had plastic bags on them in an attempt to prevent rain or snow from getting into the electrical components.
