29 CFR 1918.66(d)(4): A record of each monthly inspection was not maintained for 6 months in or on the crane, derrick or at the terminal.    a) 610 S Riverside Dr. - On or about 1/10/17, employees are exposed to struck-by hazards when monthly inspections are not documented nor maintained for the PB#1 Bucyrus-Erie Model 88B Series 4 crane.      In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
