29 CFR 1910.147(c)(2)(ii): The employer's tagout energy control program procedures did not successfully comply with 29 CFR 1910.147(c)(3) and no lockout was provided for employee protection.  For the operators of the PAC-MAC machine performing adjustments without performing lock out tag out procedures on or about December 12, 2016 and times prior there to, the employer allowed operators of the PAC-MAC machine to open the door that guards the belts and sprockets, and the point of operation of the packaging machine to adjust settings with the machine engergized.
