29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets. a. On or about January 9, 2017, located in the basement of the old Bellows Avenue Elementary School, employees were removing scrap building material while not wearing nor being provided with hardhats while other employees were working on the second and third floor where there were floor openings, thereby exposing the employees in the basement to a struck-by hazard.

29 CFR 1926.501(b)(1): "Unprotected sides and edges." Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. a. On or about January 9, 2017, located on the northern side of the second floor at the old Bellows Avenue Elementary School, employees were removing scrap material and trash from the building where there was a ten foot opening to the unprotected side of the floor at the hallway where there were preexisting classrooms where the floors had been removed and/or collapsed, thereby exposing the employees to a fall hazard of approximately 11 feet, 3 inches. b. On or about January 9, 2017, located on the northwestern side of the second floor at the old Bellows Avenue Elementary School, employees were removing scrap material and trash from the building where there was a 36 inch opening to the unprotected side of the floor at the hallway where there were preexisting classrooms where the floors had been removed and/or collapsed, thereby exposing the employees to a fall hazard of approximately 11 feet, 3 inches. c. On or about January 9, 2017, located on the northeastern side of the second floor at the old Bellows Avenue Elementary School, employees were removing scrap material and trash from the building where there was a 36 inch opening to the unprotected side of the floor at the hallway where there were preexisting classrooms where the floors had been removed and/or collapsed, thereby exposing the employees to a fall hazard of approximately 11 feet, 3 inches. d. On or about January 9, 2017, located on the 1st door on the northeastern side of the second floor at the old Bellows Avenue Elementary School, employees were removing scrap material and trash from the building where there was a 36 inch opening to the unprotected side of the floor at the hallway where there were preexisting classrooms where the floors had been removed and/or collapsed, thereby exposing the employees to a fall hazard of approximately 11 feet, 3 inches. e. On or about January 9, 2017, located on the last door on the northwestern side of the second floor at the old Bellows Avenue Elementary School, employees were removing scrap material and trash from the building where there was a 36 inch opening to the unprotected side of the floor at the hallway where there were preexisting classrooms where the floors had been removed and/or collapsed, thereby exposing the employees to a fall hazard of approximately 11 feet, 3 inches. f. On or about January 9, 2017, located on the northern side on the third floor at the old Bellows Avenue Elementary School, employees were removing scrap material and trash from the building where there was a ten foot wide opening to the unprotected side of the floor at the hallway where there were preexisting classrooms where the floors had been removed and/or collapsed, thereby exposing the employees to a fall hazard of approximately 26 feet. g. On or about January 9, 2017, located on the northwestern side on the third floor at the old Bellows Avenue Elementary School, employees were removing scrap material and trash from the building where there was a 36 inch wide opening to the unprotected side of the floor at the hallway where there were preexisting classrooms where the floors had been removed and/or collapsed, thereby exposing the employees to a fall hazard of approximately 26 feet. h. On or about January 9, 2017, located on the southwestern side on the third floor at the old Bellows Avenue Elementary School, employees were removing scrap material and trash from the building where there was a 36 inch wide opening to the unprotected side of the floor at the hallway where there were preexisting classrooms where the floors had been removed and/or collapsed, thereby exposing the employees to a fall hazard of approximately 26 feet.
