29 CFR 1926.102(a)(1): 29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles.  On or about January 11, 2017 and times prior there to, at the worksite located at or near 1915 Breckenridge Drive, Little Rock, AR. 72227.  Employees operating pneumatic nail guns did not use protective eye wear.  This exposed employees to flying debris such as, but not limited to wood chips, nails or nail fragments, and granular material from the composite shingle material.  Cesar Hernandez was previously cited for a violation of this Occupational Safety and Health Standard or its equivalent standard 29 CFR 2016.102(a)(1), which is contained in OSHA inspection 1175981, citation 1, item 1 and was affirmed as a final order on November 2, 2016 with respect to the workplace located at 3313 Crepe Myrtle Cove, Benton, AR. 72015

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about January 11, 2017 and time prior there to,  at the worksite located at or near 1915 Breckenridge Drive, Little Rock, AR. 72227.  An employee and supervisor were observed working on the roof in excess of 6 feet without the use of fall protection exposing the employees to fall hazards.  Cesar Hernandez was previously cited for a repeat violation of this Occupational Safety and Health Standard or its equivalent standard 29 CFR 1926.501(b)(13), which is contained in OSHA inspection number 1175981, citation 2, item 1 and was affirmed as a final order on November 2, 2016 with respect to a workplace located at 3313 Crepe Myrtle Cove, Benton, AR. 72015.  Cesar Hernandez was previously cited for a violation of this Occupational Safety and Health Standard or its equivalent standard 29 CFR 1926.501(b)(13), which is contained in OSHA inspection number 613818, citation 1, item 1 and was affirmed as a final order on October 23, 2012 with respect to a workplace located at 624 Pine Bluff Street in Malvern, AR. 72104.
