29 CFR 1926.304(i)(1): Each circular had-fed ripsaw was not guarded by a hood which completely enclosed that portion of the saw above the table and that portion of the saw above the material being cut. On or about January 10, 2017 and at times prior thereto, at the worksite located at 723 Central Avenue, Hot Springs, Arkansas, the guard on the hand-fed table saw did not completely enclose the top portion of the saw blade. This condition exposed employees to amputation hazards.

29 CFR 1904.39(a)(2): The employer failed to report the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, within (24) twenty-four hours: On or about December 16, 2016, the employer did not report an workplace amputation to OSHA within 24 hours. This delay caused a delay in the onsite inspection of the worksite until the injured employee reported the amputation to OSHA on January 8, 2017.
