29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: On or about January 11, 2016, at the above addressed jobsite, an employee was observed painting the side of a 4-story building from an aerial lift, without the use of a body belt and lanyard, therefore exposing the employees to a fall hazard. NOTE: CERTIFICATION AND DOCUMENTATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: On or about January 11, 2016, at the above addressed jobsite, employees were observed painting the side of a 4-story building from a balcony/ledge; and were working without the use of conventional fall protection (e.g. harness, guardrail or net), therefore exposing the employees to a fall hazard. NOTE: CERTIFICATION AND DOCUMENTATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION.
