29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:    (a) 30 West Boundary Avenue York, PA - Employee(s) were installing plywood flooring at the edge of the 2nd floor of a residential rehab home without the benefit of a fall protection system, thereby exposing employees to injuries from a fall of approximately 8 feet 6 inches to the lower level below, on or about January 11, 2017.    (b) 30 West Boundary Avenue York, PA - Employee(s) were exposed to falling through a hole approximately 4 feet wide by 4 feet long thereby exposing employees to injuries from a fall of approximately 8 feet 6 inches to the lower level below, on or about January 11, 2017.         Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
