Additional counterweighting of fork trucks was not approved by the truck manufacturer: a) At the shop yard, on January 11, 2017, a Komatsu 30 Forklift, model number FG30HT-16, serial number 202846, was altered by the addition of a 2-inch ball trailer tow not provided by the manufacturer.
