29 CFR 1910.303(g)(1): Sufficient access and working space was not provided and maintained about all electric equipment (operating at 600 volts, nominal, or less to ground) to permit ready and safe operation and maintenance of such equipment.      a)  In the plant near the Rebuild Department, there were 3 electrical panels blocked by welding equipment and cleaning supplies exposing employees to electrical and fire hazards.    b)  In the plant at the far end past the Dismantling department, 2 electrical panels were blocked by stock and by portable tools exposing employees to electrical and fire hazards.

29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed.        a)  On the east wall in the Rebuild department by the electrical panel boxes, the employer has a 4-plex metal receptacle that has an unused opening on the side not closed, exposing employees to electrical shock hazard.    b)  The control box to the "Notcher" machine has an unused opening on the front side, exposing employees to electrical shock hazard.    c)   The control box to the "Hawg" saw has an unused opening on the front side, exposing employees to electrical shock hazard.

29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure.      a)  In the Repair Department, an extension cord ran across the floor to energize another cord to energize a power tap in lieu of permanent wiring, exposing employees to electrical shock and fire hazards.      b)  In the Saw Department, several extension cords ran across the floor up underneath machinery to energize another cord to energize certain machines in lieu of permanent wiring, exposing employees to electrical shock and fire hazards.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws.   The employer failed to ensure that the orange electrical extension cord located near the "Dismantler" used to energize the saw had been pulled away at the strain relief, exposing employees to electrical shock hazards.
