29 CFR 1910.136(a): The employer did not ensure that each affected employee uses protective footwear when working in areas where there is a danger of foot injuries due to falling or rolling objects, or objects piercing the sole, or when the use of protective footwear will protect the affected employee from an electrical hazard, such as a static-discharge or electric-shock hazard, that remains after the employer takes other necessary protective measures: On or about January 10, 2017, at SpeeDee Oil Change & Auto Service, the employer failed to insure that employees use protective footwear while in the mechanic shop area of the SpeeDee Oil. Employees walked throughout the mechanic shop regularly each day and were exposed to struck-by hazards.

29 CFR 1910.305(b)(1): Openings through which conductors enter boxes, cabinets, or fittings were not effectively closed: On or about January 10, 2017, at SpeeDee Oil Change & Auto Service, the employer failed to ensure that conductor boxes were effectively closed. Employees used the automotive lift on a daily basis. The lever for raising and lowering the lift was located next to the exposed electrical wiring. Instance 1) Bay 4, 12,000 pound capacity, LDA, model R-55, electrical fittings not effectively closed.

29 CFR 1910.157(g)(1): An educational program was not provided for all employees to familiarize them with the general principles of fire extinguisher use and the hazards involved with incipient stage firefighting: On or about January 10, 2017, a SpeeDee Oil Change & Auto Repair, the employer failed to ensure that employees were trained in the use of fire extinguisher use. Employees were expected to fight incipient fires.
