The employees were framing a new residential home located in Wadsworth, Ohio. There were employees who were on the upper level setting floor joists with employees on the ground working directly under them. The employees on the ground were not wearing protective helmets; (b) On or about January 11, 2017, employees were exposed to a fall of approximately 10 feet. The employees were framing a new residential home located in Wadsworth, Ohio. The employees were on the upper level setting floor joists. The employees were not using any form of fall protection. (c) On or about January 11, 2017, employees were exposed to a fall of approximately 10 feet. The employees were framing a new residential home located in Wadsworth, Ohio. The employees were using an 8 foot supported ladder to access the upper level. The ladder did not extend at least 3 feet above its landing.
