29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: a) Locomotive Maintenance Shop - On or about and at times prior to December 5, 2016, employees who accessed elevated areas on top of various locomotives that were scheduled for maintenance and repairs, were not provided with fall arrest equipment, resulting in fall hazards of up to 8 feet.
