29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold that was more than 10' above a lower level, was not protected by a personal fall arrest system:  On or about 01/12/2017, employees at the Iroquois Street roofing project were observed working on a ladder jack scaffold system that was 14' above the ground, and they were not protected from falls by a personal fall arrest system.

On or about 01/12/2017, employees at the Iroquois Street roofing project were observed working on an improperly erected ladder jack scaffold system, exposing them to 14' fall hazards. The scaffold platforms were bridged  on to another.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  On or about 01/12/2017, employees were exposed to fall hazards in excess of 15' while engaged in roofing activities on a two story home on Iroquois Street, and they were not protected from falling by means of an approved method of fall protection.
