Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to potential crushing hazards from falling loads and/or lift parts:    (a)    On January 10, 2017 for employees who were exposed to crushing hazards where telescopic arms on ALM model 304-01195 lift did not have locking mechanisms in place while servicing vehicles at or near 500 17th Street SW in Jamestown, North Dakota.    (b)   On January 10, 2017 for employees who were exposed to crushing hazards where a cam pin and cotter pins were missing on ALM model 304-01195 lateral arms while servicing vehicles at or near 500 17th Street SW in Jamestown, North Dakota.       Abatement Note: Among other methods, one feasible and acceptable method of abatement would be to follow the requirements of ANSI/ALI ALOIM and manufacturers specification which includes, among other requirements, the following:            1.   Institute a preventive maintenance program according to the manufacturers specifications.            2.   Provide training to employees on proper use and care of the lifts.            3.   Be sure to repair damaged parts.                    4.   Schedule and complete all periodic and annual inspections.
