OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fall hazards. (a) 460 Sterling Street, Camp Hill Pennsylvania - Employees operated a Crown order picker to access items at elevated levels exposing them to fall hazards. Employees wore a body belt with a lanyard that was not energy absorbing and was tied in a knot, on or about January 12, 2017. (b) 460 Sterling Street, Camp Hill Pennsylvania An employee operated an order picker to access items from elevated levels exposing them to fall hazards. An employee who exceeded the load limit of 220 pounds, was allowed to wear a body belt and with a self-retracting lanyard device on a Yale order picker, on or about January 12, 2017. No abatement certification or documentation required.

29 CFR 1910.132(a): Protective equipment was not maintained in a sanitary and reliable condition: (a) 460 Sterling Street, Camp Hill Pennsylvania - Employees operated two order pickers to access items at elevated levels. The employer did not ensure components of the personal fall protection system were inspected and maintained in a reliable condition, on or about January 12, 2017. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
