29 CFR 1926.1053(b)(22): Employees using ladders were carrying objects or loads that could cause the employee to lose balance and fall: a) East-side of Roof- On or about January 11, 2017, employee was exposed to an approximate 30' fall to an asphalt driveway while carrying stacks of slate shingles up an extension ladder. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification. This documentation may include, but is not limited to evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a) Roof area of residential home (East-side) - On or about January 11, 2017, employee did not use fall protection to protect from falling an estimated 35 - 40 feet while installing slate shingles on a multiple pitched residential roof. The Jack's Roofing Company Inc. was previously cited for a violation of this occupational safety and health standard 29CFR 1926.501(b)(13), which was contained in OSHA inspection number 975365, citation number 1, item number 1 and was affirmed as a final order on July 15, 2014, with respect to a workplace located at 2367 King Place, Washington DC. 20016
