All energy isolating devices that are needed to control the energy to the machine or equipment shall be physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s). a. On or about February 7, 2017, located at the hub cutter, the employer used a Kirk Key system in place of an energy isolating device in order for employees to change the cutting tips on the hub cutters. It was also found that the Kirk Keys were left chained to the outside doors while the employees were inside the hub cutter area which did not allow for employees to have control over their own safety. b: On or about February 7, 2017, located in the mold repair department, employees changed out cutting tools on two King Mills from King Machine Tool Division, serial number 3291 by stopping the machine without isolating energy and performing lockout, thereby exposing the employees to a caught-in hazard from the unexpected startup of the table. c: On or about February 7, 2017, located in the mold repair department, employees changed out cutting tools on two King Mills from King Machine Tool Division, serial number 3263 by stopping the machine without isolating energy and performing lockout, thereby exposing the employees to a caught-in hazard from the unexpected startup of the table. d. Located at the Core Baker, employees used a Kirk Key system instead of an isolation device when performing service and maintenance tasks, i.e. replacing heating elements, thereby exposing the employees to the unexpected startup or release of stored energy.

29 CFR 1910.333(b)(2)(iii)(A): A lock and a tag shall be placed on each disconnecting means used to deenergize circuits and equipment on which work is to be performed, except as provided in paragraphs (b)(2)(iii)(C) and (b)(2)(iii)(E) of this section. The lock shall be attached so as to prevent persons from operating the disconnecting means unless they resort to undue force or the use of tools. a. On or about January 3, 2017, located at the Core Baker machine, a personnel change occurred during a lockout condition when in the process of removing a heating element. An employee's lock was removed from a 480 volt panel leaving a common electrician's lock that all electricians had a key to. The oncoming electrician relied on this lock for protection while other employees were able to access the electrical cabinet. The cabinet was not isolated during a lockout activity, thereby exposing the employee to an electrical shock hazard of 480 vac and/or 240 vac.

29 CFR 1910.333(b)(2)(iv)(B): A qualified person shall use test equipment to test the circuit elements and electrical parts of equipment to which employees will be exposed and shall verify that the circuit elements and equipment parts are deenergized. The test shall also determine if any energized condition exists as a result of inadvertently induced voltage or unrelated voltage backfeed even though specific parts of the circuit have been deenergized and presumed to be safe. If the circuit to be tested is over 600 volts, nominal, the test equipment shall be checked for proper operation immediately after this test. a. On or about January 3, 2017, located at the Core Baker, employees threw a 480 volt disconnect at panel 1735 to isolate the electrical energy. The employees did not verify isolation prior to performing work, thereby exposing the employees to an electrical shock and/or arc flash/blast hazard.

On or about February 8, 2017, in the maintenance shop, an employee operated the Hyd-mech horizontal band saw, Model 8-20A, Serial Number 81205785FD, that had unused portions of the blade not guarded, thereby exposing the employee to an amputation hazard. b. On or about February 7, 2017, in the maintenance shop, an employee operated the Marvel Series 8 Mark 1 vertical band saw, Serial Number 821613-W, that had unused portions of the blade below the saw table not guarded, thereby exposing the employee to an amputation hazard. c. On or about February 7, 2017, in the maintenance shop, an employee operated the Marvel Series 8 Mark 1 vertical band saw, Serial Number 821613-W, that had the unused rear blade above the saw table not adequately guarded, thereby exposing the employee to an amputation hazard. d. On or about February 7, 2017, in the maintenance shop, an employee operated an Atrump vertical manual mill, Model K6F, Serial Number K6F-153, that did not have a chip guard to prevent contact with the flying chips, exposing the employee to a struck-by hazard. e. On or about February 7, 2017, located at the conveyor turnstyle southeast of the core baker, there was a set of stairs that did not have adequate guarding to protect employees walking through the area from falling into the turnstyle, thereby exposing the employees to a caught-between hazard. b. On or about February 7, 2017, located at the drag cleaner, there was an opening near where employees walked and where an operator would stand that did not prevent an employee from falling into the chute located under the drag cleaner, thereby exposing the employees to fall-related injuries. The Amsted Rail Company Inc. was previously cited for a violation of an equivalent occupational safety health standard 1910.23(c)(3), which was contained in OSHA inspection number 1030504, citation number 1, item number 1 and was affirmed as a final order on October 6, 2016, with respect to a workplace located at 2100 Griffin Wheel Drive, Bessemer, AL 35020.

29 CFR 1910.147(c)(4)(ii): The procedures shall clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, the following: a. At the workplace located at 3900 Bixby Road in Groveport, Ohio, the specific energy control procedure for the East Core Baker Shuttle Pad did not contain the necessary information, to include but not limited to, the following when employees replaced heating elements: (1) Specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy pursuant to 1910.147(c)(4)(ii)[B]. The procedure did not address controlling the hazardous energy sources by first shutting the equipment down, addressing heat and 240 volts as energy sources. (2) Specific procedural steps for the placement, removal and transfer of lockout devices or tagout devices and the responsibility for them pursuant to 1910.147(c)(4)(ii)[C]. The procedure was not specific to the location and energy isolating device to be used, such as, the specific location of the ball valve and the 480 volt electrical panel disconnect switch with the 240 volts being supplied to the pads. 29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc. a. On or about February 7, 2017, located at the upender near the Hot Wheel Grinder, there was a pneumatic lift that lifted railroad wheels. An elbow fitting came within approximately three inches from a fixed railing when its most upward position, exposing the employees to a caught-between hazard. b. On or about February 7, 2017, located at middle foundry inspection table, there was an automated conveyor system that took wheels from an elevator to a lower conveyor. There was an opening of approximately 13 inches when the elevator was at its upper limit. When the elevator lowered a pinch point was created between the elevator and the fixed conveyor body, exposing the employees to a caught-in/caught-between hazard. c. On or about February 7, 2017, located between the Hot Wheel Kiln and upender on the conveyor line, there was a shuttle car that would handle wheels from each conveyor line. The outer portion of the railed car was inadequately guarded and would get within approximately three inches from the guardrails toprail and four inches from the vertical steel column, exposing the employees to a caught-between hazard. d. On or about February 7, 2017, located at the turnstyle that was east of the Drag Cleaner, there was an 18-1/2 inch gap in guarding that did not prevent employee exposure to the rotating turnstyle, thereby exposing the employees to a caught-between hazard. e. On or about February 7, 2016, located on the back side of the hub cutters, there was a conveyor line that transported wheels with unguarded powered rollers, thereby exposing employees to caught-in hazards of the rollers. f. On or about February 8, 2017, in the final inspection booth, an employee operated the wheel turning mechanism that had a pinch point between its frame and a fixed metal desk, exposing the employee to a struck by and caught-in hazard. The Amsted Rail Company was previously cited for a violation of this occupational safety and health standard 1910.212(a)(1), which was contained in OSHA inspection number 1028084, citation number 1, item number 1 and was affirmed as a final order on (date), with respect to a workplace located at 7111 Griffin Road Kansas City, KS 66111.

On or about February 7, 2017, in the maintenance shop, an employee operated the Marvel Series 8 Mark 1 vertical band saw, Serial Number 821613-W, without guards that prevented the operators arms, hands, or fingers from entering the danger zone during the operation of cutting an approximately one inch diameter chrome rod, thereby exposing the employee to an amputation hazard. b. On or about February 7, 2017, located in the mold repair department, employees operated a King Mill from King Machine Tool Division, serial number 3291 that did not have guarding to prevent employee contact with the point-of-operation, exposing the employees to an amputation hazard. c. On or about February 7, 2017, located in the mold repair shop, employees operated a King Mill from King Machine Tool Division, serial number 3263, that did not have guarding to prevent employee contact with the point-of-operation, exposing the employees to an amputation hazard. On or about January 12, 2017, located between the Hot Wheel Kiln and upender on the conveyor line, there was a shuttle car that would handle wheels from each conveyor line. On the side of the shuttle car there was an unguarded chain and sprocket that was 8-1/2 inches from a fixed open guardrail, thereby exposing the employees to a caught-in hazard. Safety signs, safety symbols, or accident prevention tags shall be used where necessary to warn employees about electrical hazards which may endanger them, as required by 1910.145. a. On or about January 12, 2017, located at the Core Baker, there were four pads that employees were required to clean by hand after each heat. In the center of each pad was a cover with a handle that could be removed by hand where there were two live electrical rings with 240 volts present. There were no safety signs, symbols or tags present to alert employees of the electrical hazard, thereby exposing the employees to an electrical shock hazard.
