29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about January, 12, 2017, the employer had 13 ACME-GRIDLEY multi-spindle screw machines. Eight of the machines were in operation, numbers 1, 2, 3, 4, 5, 6, 8 and 11. The slide door guarding on both sides of the tooling areas were open and the slide doors did not have interlocks. The rotating drive axle shaft ends were not adequately guarded. The metal chip augers for machines numbered 2 and 11 were not guarded.

The openings between the periphery of the abrasive wheels and the shroud was 3/4 of an inch.

29 CFR 1910.305(b)(1)(i): Conductors entering cutout boxes, cabinets, or fittings were not protected from abrasions: On or about January 12, 2017, the numbers 2, 4 and 5 Acme-Gridley multi spindle screw machines had electrical wiring that was not protected from abrasion. 1. The #2 Acme-Gridley multi spindle screw machine had electrical wiring for the cam limit assembly. The flexible conduit covering the wiring was damaged and separated subjecting the wiring to abrasion. 2. The #4 Acme-Gridley multi spindle screw machine had electrical wiring for the starter switch to the metal chip conveyor. The conduit fitting was separated at the side of the starter switch subjecting the wiring to abrasion. 3. The #5 Acme-Gridley multi spindle screw machine had electrical wiring for the main disconnect switch. The flexible conduit covering the wiring was damaged and separated subjecting the wiring to abrasion.
