29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:         a.)  Thiel College, 75 College Ave., Greenville, PA, on or about January 12th, 2017 and times prior thereto:  Employees worked with and around liquid chemicals including, but not limited to, AirX 44ACE and Air X 60,  during cleaning operations onsite.  Employees were not required to wear appropriate eye protection during work operations with and around these hazardous substances.

29 CFR 1910.1030(c)(1)(ii)(B):  The employer's Exposure Control Plan did not include the schedule and method of implementation for paragraphs (d) Methods of Compliance, (f) Hepatitis B Vaccination and Post-Exposure Evaluation and Follow-up, (g) Communication of Hazards to Employees, and (h) Recordkeeping, of this standard:       a) Thiel College, 75 College Ave., Greenville, PA, on or about January 12th, 2017 and times prior thereto:  Employees had potential exposure to bloodborne pathogens intermittently throughout work operations including, but not limited to, cleaning up blood or other potentially infection material (OPIM) during housekeeping work activities.  The employers written Bloodborne Pathogen Exposure Control Plan was not site specific in that, the written plan did not identify items including, but not limited to:          1)  An exposure determination to determine which job classifications have occupational exposure to blood or OPIM.     2)  The listed task/procedures in which these job classifications would have occupational exposure to blood or OPIM.    3)  The schedule and method of implementation of the hepatitis B Vaccination and Post-Exposure Evaluation and Follow-up.    4)  Additional information regarding the communication of hazards to employees.  Specifically, the method and implementation of training regarding bloodborne pathogens and OPIM for employees with occupational exposure.   5)  The Exposure Control plan was not reviewed and updated at least annually and whenever necessary to reflect new modified tasks and procedures which affect occupational exposure.

29 CFR 1910.1030(g)(2)(ii)(B): The employer did not ensure that the training was provided to employees with occupational exposure at least annually:     a) Thiel College, 75 College Ave., Greenville, PA, on or about January 12th, 2017 and times prior thereto:  Employees had potential exposure to bloodborne pathogens intermittently throughout work operations including, but not limited to, cleaning up blood or other potentially infection material (OPIM) during housekeeping work activities.  Some employees who had potential occupational exposure to blood/OPIM were not provided with annual bloodborne pathogen training.

The employer did not make their Hazard Communication Program available upon request in accordance with 29 CFR 1910.1020 (e).
