29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  (a)On January 11, 2017, at a jobsite at 2258 SW Wall Street, Blue Springs, Missouri, employees were exposed to a fall hazard of approximately 15 feet, in that they were conducting roofing work on a steep roof without any form of fall protection.  Abatement certification is required for this item; use the CERTIFICATION OF CORRECTIVE ATION WORKSHEET for this purpose.

29 CFR 1926.503(a)(2)(iii): The employer did not assure that each employee has been trained, as necessary, by a competent person in the use and operation of guardrail systems, safety net systems, warning line systems, safety monitoring systems, controlled access zones, and other protection to be used.  (a)On January 11, 2017, at a jobsite at 2258 SW Wall Street, Blue Springs, Missouri, employees were exposed to a fall hazard of approximately 15 feet while conducting roofing work on a steep roof, in that they had not been trained in the use and operation of required Fall Protection Systems.  Abatement certification is required for this item; use the CERTIFICATION OF CORRECTIVE ATION WORKSHEET for this purpose.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  (a) On January 11, 2017, at a jobsite at 2258 SW Wall Street, Blue Springs, MO, employees were exposed to fall hazards of approximately 10 feet and 15 feet. Employees were accessing working surfaces on a steep pitch roof by way of portable ladders whose side rails did not extend at least 3 feet above the upper landing surface being accessed.    Abatement certification is required for this item; use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.
