29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: (a) Colorado Seamless Gutters, Inc. at 6255 Habitat Dr, Boulder, CO 80301: On or before January 12, 2017, the employer did not ensure that employees were protected from injury, in that employees accessing the edge of the roof did not utilize a personal fall arrest system, when cleaning gutters on a three story apartment building. This condition exposed employees to a fall of approximately 28 feet.
