29 CFR 1910.212(a)(1):  Machine guarding was not provided to protect operator(s) and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:                                                                                                                The employer does not ensure all equipment used at the facility is provided with proper machine guarding, exposing employees to crush-by and amputation hazards.        a)   This violation occurred on or about November 14, 2016, where a Floor Model C Frame hydraulic press was not equipped with effective guards or safeguarding to protect employees from point of operation hazards and a cheater bar was used to allow employees to by-pass the two hand controls.    b)   This violation was observed on or about November 23, 2016, where a Standard 200 ton hydraulic press was not equipped with light curtains or other machine guarding to protect employees.      c)   This violation was observed on or about November 23, 2016, where a HTC hydraulic press was not equipped with light curtains or other machine guarding to protect employees.      Contract Manufacturer dba CM Trailers was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1910.212(a)(1), which was contained in OSHA inspection number 316238658, citation number 1, item number 6, and was affirmed as a final order on October 17, 2012, with a final abatement date of September 28, 2012, with respect to a workplace hazard at 1102 N. Industrial Road Madill, OK 73446.       Contract Manufacturer dba CM Truck Beds was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1910.212(a)(1), which was contained in OSHA inspection number 316238732, citation number 1, item number 2, and was affirmed as a final order on October 17, 2012, with a final abatement date of September 28, 2012, with respect to a workplace hazard at 1102 N. Industrial Road Madill, OK 73446.
