29 CFR 1910.107(b)(5)(iv): Space within the spray booth on the downstream and upstream sides of filters were not protected with approved automatic sprinklers:       On or about July 13, 2016 and at times prior there to, overspray dry filters in the COL-MET spray booth(s) located in the powder coating area(s) were not protected by approved automatic sprinklers on the upstream and downstream side of the filters.   Employees are exposed to fire and explosion hazards during manual electrostatic powder spraying operations.     Pursuant to 29 CFR 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation describing the steps taken to ensure sprinklers are provided upstream and downstream of the spray booth filters to protect employees from hazards created by  fire and explosions.

29 CFR 1910.134(c)(1)(vi): The written program did not contain procedures to ensure adequate air quality, quantity, and flow of breathing air for atmosphere-supplying respirators:    On or about July 13, 2016 and at times prior thereto, the employers written respiratory protection program did not address procedures to ensure the quality of breathing air provided by the Quincy QSI-370 compressor used to supply breathing air to employees using supplied air respirators. The employees were exposed to inhalation hazards of including, but not limited to: carbon monoxide, water vapor, oil/oil mists. 29 CFR 1910.134(i)(1)(ii): Breathing air did not meet requirements for Grade D breathing air as described in the ANSI/Compressed Gas Association Commodity Specification G-7.1-1989:     On or about July 13, 2016, and at times prior thereto, the employer did not ensure breathing air supplied by an oil lubricated air compressor for tight-fitting elastomeric full face respirators used by employees meet the requirements for Grade D breathing air.  The employees were exposed to inhalation hazards of including, but not limited to: carbon monoxide, water vapor, oil/oil mists.     Pursuant to 29 CFR 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation describing the steps taken to ensure compressed air used for respirators meets the requirements of the United States Pharmacopeia for breathing oxygen to protect employees from respiratory injury or death.

29 CFR 1910.1000(a)(2): Employee(s) were exposed to an airborne concentration of Respirable Dust and Total Dust listed in Table Z-1 in excess of the 8 hour Time Weighted Average concentration of 5mg/m3 and 15mg/m3, respectively.     Powder coating area(s). On or about July 27, 2016 8 -hour time-weighted average (8-Hr TWA) personal sampling was performed.     a) An employee's 8-Hr TWA exposure to Respirable Dust was 15.0 mg/m3 during electrostatic hand spraying of powder coatings. Personal sampling was conducted for 455 minutes. The remaining 25 minutes were averaged in the TWA as zero. This exposure exceeded the OSHA 8-Hr TWA by 2.8 times.      b) An employee's 8-Hr TWA exposure to Total Dust was 420.0 mg/m3 during electrostatic hand spraying of powder coatings. Personal sampling was conducted for 462 minutes. The remaining 18 minutes were averaged in the TWA as zero. This exposure exceeded the OSHA 8-Hr TWA by 26.97 times.     c) An employee's 8-Hr TWA exposure to Total Dust was 140.0 mg/m3 during electrostatic hand spraying of powder coating. Personal sampling was conducted for 420 minutes. The remaining 60 minutes were averaged in the TWA as zero. This exposure exceeded the OSHA 8-Hr TWA by 8 times.       Pursuant to 29 CFR 1903.19, within thirty (30) calendar days of the date of this citation, the employer must submit documentation describing the steps taken to ensure employees are not overexposed to airborne concentrations of Respirable Dust and Total Dust to protect employees from hazardous airborne chemicals.
