29 CFR 1926.304(h)(1): Each circular crosscut table saw was not guarded by a hood which completely enclosed that portion of the saw blade above the table and that portion of the saw above the material being cut:  (a) North side of house - On or about January 11, 2017 employees were using 10 in. table saw to cut wood siding with the blade guard removed exposing the top of the saw blade.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   (a) North and East side of house - On or about January 11, 2017 employees were attaching wood siding to the face of the house wall while working on top of the front porch cover that is over 34 feet high with no means of fall protection or safety fall procedures to prevent employees from falling off the roof and striking the ground below.

(a) East side of house - On or about January 11, 2017 employees applying wood siding were working from wood scaffolding without being trained by a qualified person to recognize the hazards and how to minimize those hazards.  Juan Palomares was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.454(a) which was contained in OSHA inspection number 986247, citation number 1, item number 4 and was affirmed as a final order on 10/29/2014, with respect to a workplace located at E Scenic Dr. Pass Christian, MS. 39565.

Juan Palomares was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926. 503(a)(1), which was contained in OSHA inspection number 986247, citation number 1, item number 5 and was affirmed as a final order on October 29, 2014, with respect to a workplace located at E Scenic Dr. Pass Christian, MS. 39571.

29 CFR 1926.404(b)(1)(iii)(C): Where an assured equipment grounding conductor program was being utilized, the employer did not ensure that each cord set, attachment cap, plug and receptacle of cord sets, and any equipment connected by cord and plug, except cord sets and receptacles which are fixed and not exposed to damage, were not visually inspected fofore each day's use for external defects, such as deformed or missing pins or insulation damage, and for indicationsof possible internal damage.  (a) North side of house - On or about January 11, 2017 employees using table saw to cut siding material was using an extension cord that was damaged and repaired using non insulating electrical tape.
