29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered:    Big Ox Energy - Siouxland, LLC facility - 1617 D Avenue - Dakota City, NE - Employer is failing  to ensure protective equipment is used when fall protection hazards are present. On or about January 12, 2017, an Anthony P&L, Inc. truck driver was injured from a fall of approximately 10 feet while attempting to open the top hatch of a pressurized tanker. The employee suffered back, knee, and laceration injuries.
