29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards:  Note:  Toeboards are only required when persons can pass, there is moving equipment or there is equipment with which falling materials could create a hazard.    a) On or about 01/12/17 in the Chilling Tunnel area; employees who performed work on top of the Chilling Tunnel were exposed to a fall hazard of approximately 10 feet when there was a 40 inch opening to the railing on the SW side of the work platform.     b) On or about 01/12/17 in the Chilling Tunnel area; employees who performed work on top of the Chilling Tunnel were exposed to a fall hazard of approximately 10 feet when there was a 14 inch opening to the railing on the SE side of the work platform.      NO ABATEMENT CERTIFICATION REQURIED

It may be made of any substantial material either solid or with openings not over 1 inch in greatest dimension.  a) On or about 01/12/17 in the Chilling Tunnel area; employees who walked and worked below the Chilling Tunnel work platform, while repairs and maintenance was occurring on top of the Chilling Tunnel, were exposed to a struck by hazard when the bottom of the metal toe board was 4 inches above the platform surface. 29 CFR 1910.147(c)(4)(i): Procedures were not documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    a) On or about 01/12/17 in the tumbler area; Sanitation crew employees were exposed to an amputation hazard when they hand cleaned the Tumbler while the tumbler basket was rotating and their hand was in proximity to the drive wheel.  They did not use a procedure to control the hazardous energy.           b) On or about 01/12/17 in the packing area; Sanitation crew employees were exposed to an amputation hazard when they hand cleaned the conveyors inside the Chilling Tunnel while the conveyor was operating without using a procedure to control the hazardous energy.       c) On or about 01/12/17 in the packing area; Production employees were exposed to an amputation hazard when they changed over the Ishita rotary filler by removing and installing the baskets without using a written procedure to control the two hazardous energy sources ( air and electricity).        d) On or about 01/12/17 in the packing area; Sanitation crew employees were exposed to an amputation hazard when they hand cleaned the baskets on the Ishita rotary filler without using a written procedure to control the two hazardous energy sources ( air and electricity).        e) On or about 01/12/17 in the raw area; Sanitation crew employees were exposed to an amputation hazard when they hand cleaned the inside portions of the raw conveyor without using a procedure to control the hazardous energy.        f) On or about 01/12/17 in the tumbler area; Sanitation crew employees were exposed to an amputation hazard when they hand cleaned the wear strips and sprockets of the scale incline conveyor without using a procedure to control the hazardous energy. 29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation;     a) On or about 01/12/17 in the tumbler area; Sanitation crew employees were exposed to an amputation hazard when they cleaned the Tumbler while the tumbler basket was rotating and their hand was in proximity to the drive wheel without being trained to shut off the machine prior to cleaning and control the hazardous energy.        b) On or about 01/12/17 in the packing area; Sanitation crew employees were exposed to an amputation hazard when they hand cleaned the conveyor inside the Chilling Tunnel while the conveyor was operating without being trained to shut off the conveyors prior to cleaning and control the hazardous energy.        c) On or about 01/12/17 in the packing area; Production employees were exposed to an amputation hazard when they changed over the Ishita rotary filler without being trained to shut off the air and main power prior to opening the access door and removing and installing the baskets.       d) On or about 01/12/17 in the packing area; Sanitation crew employees were exposed to an amputation hazard when they hand cleaned the baskets on the Ishita rotary filler without being trained to shut off the air and main power prior to opening the access prior to this activity.           e) On or about 01/12/17 in the raw area; Sanitation crew employees were exposed to an amputation hazard when they hand cleaned the inside portions of the raw conveyor without being trained to control the hazardous energy of the machine prior to cleaning.      f) On or about 01/12/17 in the tumbler area; Sanitation crew employees were exposed to an amputation hazard when they hand cleaned the wear strips and sprockets of the scale incline conveyor without being trained control the hazardous energy of the prior to cleaning    ABATEMENT DOCUMENTATION REQUIRED
