On or about December 8, 2016, an employee fell from an extension ladder that was displaced while the employee worked from the ladder.

29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards: (a) The employer did not ensure that ladder safety training had been conducted with employees who work from ladders. On or about December 8, 2016, there was an accident in which an employee fell from an extension ladder that was not secured to prevent movement or displacement.
