29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energy, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: On or about January 10, 2017 at the AJM Packaging Facility- employees were exposed to serious amputation, crushing and struck-by injuries, in that the employer had not developed an Energy Control Program for the various types of machinery and equipment. Abatement certification is required for this item; use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about January 4, 2017 at the AJM Packaging facility- Employee was exposed to pinch/nip points from the paper baler machine, in that the machine was activated while employee's fingers were exposed and not covered or guarded to prevent nip/pinch points from contacting the employee. Abatement verification for this item is required.
