Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to potential crushing hazards from falling loads and/or lift parts:  (a) On January 13, 2017 for employee who was exposed to crushing hazards where telescopic arms on Hanmecson model Pro 12 lift  did not have locking mechanisms in place while servicing vehicles at or near 1345 West Main Street North in Valley City, North Dakota.  (b) On January 13, 2017 for employee who was exposed to crushing hazards where lateral lift arms on Bend Pak lift model MX-10ACX did not have locking mechanisms in place while servicing vehicles at or near 1345 West Main Street North in Valley City, North Dakota.    Abatement Note: Among other methods, one feasible and acceptable method of abatement would be to follow the requirements of ANSI/ALI ALOIM and manufacturers specification which includes, among other requirements, the following:          1.   Institute a preventive maintenance program according to the manufacturers specifications.       2.   Provide training to employees on proper use and care of the lifts.       3.   Be sure the lifts locking device is engaged and working.       4.   Schedule and complete all periodic and annual inspections.
