29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed: On or about December 21, 2016, in the Tubing Department, the employer failed to conduct periodic inspections and Supervision to ensure correct energy isolation procedures for the Logan hydraulic tube forming Press #2 were followed. Failure to do so exposed employee to caught-in and amputation hazards.

Failure to do so exposed employees to pinch point and amputation hazards.
