29 CFR 1926.451(a)(6): Scaffolds were not designed by a qualified person. On 14 January 2017, two employees were installing sheetrock over a stairway while utilizing a makeshift scaffold that had not been designed by a competent person, exposing both workers to a fall hazard in excess of 10 feet.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level. On 14 January 2017, two employees were installing sheetrock above a stairway while standing on a scaffold platform without fall protection, exposing the workers to a fall hazard of approximately 10 feet 5 inches to the stairway landing or 14 feet 1 inch to the slab below.
