29 CFR 1910.176(c): Storage areas were not kept free from accumulation of materials that constituted hazards from tripping, fire, explosion or pest harborage Store Room, 50 Holyoke Street, Holyoke, MA: On or about January 13, 2017 employees were observed working near and around a store room which was not free from an accumulation of materials that constituted hazards from tripping, fire and explosion or pest harborage exposing employees to struck hazards, slips, trips and falls. (a) Fixture room, tripping hazards from debris and shelving on floors and in aisles. (b) Elevator Control room entrance, tripping hazards from pallets, pallet jack and other debris blocking aisle leading and in front of elevator control room door. (c) Compactor Control panel, operating panels and other electrical equipment blocked by carts and other rubbish. Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: Store room, 50 Holyoke Street, Holyoke, MA: On or about January 13, 2017 employees were observed working near or around extension cords and charging cords which were plugged into live outlets without being connected to the equipment they were designed to power against listed and labeled requirements by the manufacturer. (a) Cord, charging, automatic pallet jack, 2nd floor store room. Power cord plugged into wall and not connected to equipment. (b) Cord, charging, automatic pallet jack, first floor store room. Power cord plugged into wall and not connected to equipment.
