29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.       a) 658 Ridgewood Rd., Maplewood, NJ      Employees were exposed to an approximately 9-foot fall hazard to the ground as the ladder side rails did not extended at least 3 feet above landing surface.        Violation was observed on or about 12/30/2016.

Violation was observed on or about 12/30/2016.      The employer was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 925825, citation number 1, item number 2 and was affirmed as a final order on 08/19/2013, with respect to a workplace located at 28 Burnett Place, West Orange, NJ 07052.
