29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.       a) 658 Ridgewood Rd., Maplewood, NJ      Employees were exposed to an approximately 9-foot fall hazard to the ground as the ladder side rails did not extended at least 3 feet above landing surface.        Violation was observed on or about 12/30/2016.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):       a) 658 Ridgewood Rd., Maplewood, NJ      Employees performing roofing work on multi-unit residential buildings approximately 10-feet above the ground surface were not protected from fall hazards.        Violation was observed on or about 12/30/2016.      The employer was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 925825, citation number 1, item number 2 and was affirmed as a final order on 08/19/2013, with respect to a workplace located at 28 Burnett Place, West Orange, NJ 07052.
