29 CFR 1910.28(b)(2)(ii): When any portion of a guardrail system, gate, or chains were removed, and an employee was leaning through or over the edge of the access opening to facilitate hoisting, the employee was not protected from falling by a personal fall arrest system: (a) On or about January 10, 2017, employees were exposed to fall hazards in excess of four feet in that during hoisting operations of the High Performance Amplifier (HPA), personal fall arrest system(s) were not used after removal of a platform gate. Exposure to this hazard resulted in an employee falling approximately 21 feet to the ground. Abatement Note: Abatement certification and documentation are required for this item (see enclosed "Certification of Corrective Action Worksheet").
